Final Report on BAe 125-800B Aircraft, 5N-BOO

Accident Investigation Bureau (AIB) was notified of the incident by the Nigerian Civil Aviation Authority (NCAA) on 16th July, 2020. Investigators were dispatched to the incident site the following day and commenced post occurrence assessments.

At 17:00:15 h and about 20 NM to POT, when 5N-BOO was lined up as the first in the landing sequence, aural and visual ENGINE 1 FIRE warnings came ON. The flight crew executed ENGINE FIRE IN FLIGHT checklist, including retarding the throttles, shutting the fuel supply and firing extinguisher bottle. While still executing the ENGINE FIRE IN FLIGHT checklist, another ENGINE FIRE warning came ON. The flight crew declared MAYDAY1 and then continued with the checklist, including firing the second extinguisher bottle. The MAYDAY declaration was not copied by the ATC. Shortly afterwards, Approach Control (APP) requested to know if operations were normal to which the flight crew said that they declared MAYDAY on account of “engine troubles”. Emergency procedures were activated by Tower. At 17:11:15 h, 5N-BOO landed safely.
